Care gaps

Limited rollout

Bring bone-health care gaps into the clinic visit.

Patients with osteoporosis risk are often already on the orthopedic schedule. Bone Health flags them before the visit and prepares a referral the physician can sign or set aside.

The problem

The visit is the best chance to act.

Fragility-fracture risk is easy to miss in a busy clinic, and follow-up depends on someone noticing it during the visit. A flag that arrives with the schedule gives the physician a chance to decide while the patient is in the room.

How it works

Prepared by OpAgent. Decided by your team.

  1. 01

    What OpAgent prepares

A flag on the clinic schedule for patients whose chart shows bone-health risk factors, such as age or a prior fragility fracture, with the reasons listed. When the practice's referral criteria are met, OpAgent prepares a draft referral to the practice's bone-health program. When they are not, the flag notes that screening may be worth considering and no order is drafted.

  2. 02

    What the physician reviews

The physician reviews the flag and the chart evidence, then decides whether a workup is appropriate. A missing bone-density result does not on its own mean a scan is indicated.

  3. 03

    Where approved work goes

If the physician agrees, they sign the referral in athenaOne. If not, nothing is ordered.

Evidence

What has been measured

Measured

201

Unique patients flagged for bone-health review

Population
Patients on the clinic schedule at one participating orthopedic practice
Sample
201 unique patients
Period
Four clinic days, August 2026 benchmark
Compared with
None. This is a count of flags, not a comparison.

Limitation: A flag asks the physician to consider a bone-health workup. It is not an order, a referral, or a completed visit.

Measured

39 of 40

Bone-health flags supported by the chart on review

Population
Bone-health flags at one participating orthopedic practice
Sample
40 flags in a fixed review sample: 39 supported, 0 unsupported, 1 with too little information to decide
Period
August 2026 benchmark
Compared with
Chart review of each flagged patient

Limitation: A small sample. It shows whether each flag was supported by the chart, not whether care followed.
